Community Hospital presents a comprehensive program designed to provide
skills and knowledge necessary to lead a healthy life with diabetes.
Recent findings in diabetes research demonstrate that controlling blood
glucose levels can prevent or prolong the onset of diabetes-related
complications. Unfortunately, diabetes is the number one cause of new
blindness, kidney disease and amputation. The diabetes self-management
training program covers the entire array of topics needed by an individual
for the care of their diabetes.
Upon admission to the training program, an assessment is completed for
each participant and the program is adapted to your individual needs.
Family participation is encouraged, and the schedule of classes and
individual sessions is flexible to accommodate participants. With the
training, skill development and education this program will provide, you
will be able to positively effect your blood sugar control.
The comprehensive program consists of group classes and individual
sessions with a certified diabetes educator, a registered dietitian,
physical therapist and a social worker.
Patients and their families learn the "survival skills" of diabetes
self-management including: insulin injections and medications, meal
planning, blood glucose monitoring, foot care and more.
