Hospice Care
Hospice care is for any person who has a life-threatening or terminal
illness. The Visiting Nurses Association of Central Jersey (VNACJ) and
Bayshore have a longstanding commitment of working together to provide
the best care to meet our patient’s needs. By having VNACJ intake staff
on site at Bayshore, transition to hospice care can be arranged quickly
and easily whether the patient is remaining in the hospital, a facility
or transitioning home.
"Having the VNACJ stationed at the hospital is another way we can offer
comprehensive care to our patients," says Raimonda Clark, President and
CEO. "Through this partnership our patients can rest assured their
hospice care and medical needs will be met once they are home. Our new
service includes hospice in the hospital and Bayshore Health Care and
Rehabilitation Center."
Through the use of a family-centered approach, VNACJ hospice care
focuses on the comfort and dignity of the patient and support of the
family. Care is provided by an interdisciplinary team that usually
includes our doctors, a hospice nurse, a home health aide, a social
worker, physical, speech, or occupational therapist if needed, a
spiritual counselor and specially trained volunteers. Families often
consider a hospital or nursing home environment for this care when home
care is not sufficient.
